As a New or Renewing Fellow of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT), I attest that:

1. I have met all requirements to be a new member or I am currently a member in good standing and desire to obtain or continue membership status in the AAOMPT;
2. I will follow and practice within the parameters of, and agree to uphold, the AAOMPT Policies and AAOMPT Position Statements;
3. I am currently a member of and will continue to be a member of the American Physical Therapy Association (APTA);
4. I will abide by the APTA code of ethics;
5. I will provide orthopaedic manual physical therapy care consistent with current best evidence;
6. I will continue to execute professional development activities in accordance with AAOMPT standards; and
7. I agree to pay my annual Fellow membership fees.
